A template leaves the adding up to you. The generator at the top of this page does it for you, works out the GST and heads the document correctly, which is the whole reason it exists.

No GST because the business is not registered, so the document is headed "Invoice", not "Tax invoice". An unregistered business may not use that heading: it tells the customer they can claim GST that was never charged.

The call-out fee carries GST like everything else on the invoice. It is part of what you are charging for the service, not a separate GST-free item.
